A 70-year-old female with reduced ejection fraction and complex multivessel coronary artery disease (LM stenosis 30%, proximal LAD 70% with severe calcification, proximal LCx 70%) underwent successful revascularization using Impella CP for hemodynamic support.

Given the high-risk profile and heavy calcified burden, Impella CP from J&J Heart Recovery ensured circulatory stability throughout the procedure.

The intervention involved PCI from LM to LCx with intravascular lithotripsy (Shockwave 3.5 × 12 mm), DES placement (3.5 × 20 mm), and post-dilation (3.5 × 15 mm).
Proximal LAD PCI included predilation, DES implantation (3.0 × 28 mm), and post-dilation, followed by final kissing balloon inflation in LAD and LCx.

The procedure achieved an excellent angiographic result with TIMI 3 flow.
